YOU HAVE THE POWER TO DONATE LIFE!
FOR YOUR INFORMATION
Many people believe that if they carry a donor card or
at one time designated "donation" on their driver’s
license they have already registered to be an organ donor.
This is not true for Arizona residents.
The only way to register to be an organ donor is by
signing up on the Arizona Donor Registry. You can
register by going online to
www.ServiceArizona.com
and clicking the Donate Life logo or by calling 1-800-94-DONOR. Your personal information will be kept
secure and confidential.
